We use this information to assess your suitability for our rehabilitation programs, coordinate your care with your treating doctors and allied health providers, confirm funding and billing arrangements with your health fund, DVA, or insurer, and administer appointments and admissions.

How We Store and Protect It

Form submissions are transmitted securely and stored in Australia. Access is limited to clinical and administrative staff who need it to provide your care. We do not sell or use your information for marketing without your separate consent.

Sharing Your Information

We may share relevant clinical details with your referring GP or specialist, treating hospital, allied health providers involved in your care, and your private health fund, Medicare, DVA, or WorkCover/CTP insurer where relevant to your claim.
