At New Hope Rehabilitation, we work in close partnership with General Practitioners, Orthopaedic Surgeons, Neurologists, and Allied Health Clinicians across South East Queensland. We provide structured, timely assessments and multidisciplinary care pathways across outpatient, day-program, and inpatient hospital settings.
We prefer and prioritize secure electronic messaging (Medical Objects / HealthLink) to ensure rapid triage and prompt appointment scheduling. Standardized referral templates compatible with Best Practice, MedicalDirector, and Genie are available on request.
What Happens After You Refer
The same three steps your patient sees on our site, from their side of the process:
Step 1: Speak with Your Doctor
Under Australian Medicare regulations, you need a valid referral from a registered Medical Practitioner (GP or specialist) to claim Medicare rebates for your consultation. GP referrals are typically valid for 12 months; specialist referrals are typically valid for 3 months.
Step 2: Sending Your Referral
Ask your GP to send the referral directly to us via Medical Objects, secure fax, or email. Alternatively, you can upload a copy using our secure online referral form.
Step 3: We Contact You
Once Dr Iyer reviews your referral, our reception team will contact you directly to confirm your appointment time, provide intake paperwork, and outline what to bring.
What to Include in Your Referral
- Clinical Summary: reason for referral, current symptoms, functional baseline, and primary goals
- Relevant Diagnostics: recent imaging reports (X-Ray, MRI, CT), pathology, or specialist correspondence
- Medical Background: current medication regimen, allergies, and past surgical/medical history
- Funding / Care Scheme: Medicare details, private health insurance fund, DVA status, or WorkCover claim number
